Аннотация
Идентификация генетических локусов и биохимических маркеров, ассоциированных с количественными и качественными признаками костной ткани, риском переломов и уровнем минеральной плотности костной ткани, до сих пор не дала однозначного ответа на вопрос о молекулярном патогенезе остеопороза. Всё еще остаются нерешенные и вопросы, касающиеся разработки методов ранней диагностики и прогнозирования течения данного заболевания. Молекулярные эффекты генетических вариантов, расположенных в кодирующих областях генома человека, легко поддаются изучению, однако, большинство однонуклеотидных полиморфных локусов, ассоциированных с восприимчивостью к остеопорозу, локализованы в некодирующих или межгенных областях и их роль в патогенезе данного заболевания не изучена в полной мере. Использование биохимических маркеров в диагностике и мониторировании терапии остеопороза не позволяют разработать подходы к ранней диагностике заболевания до возникновения перелома. Возникают значительные проблемы интерпретации результатов исследований для применения в клинической медицине. Однако объединение многопрофильных данных, таких как полногеномный поиск ассоциаций (GWAS), изменение паттернов биогенных элементов костного ремоделирования, каталитической активности ряда ферментов и экспрессии генов, являющихся маркерами костной резорбции, позволило значительно расширить понимание ключевых звеньев патогенеза заболевания. В статье приведены обзор и обобщение результатов мультиомиксных исследований остеопороза, в том числе бионформатического анализа по поиску ключевых факторов риска развития остеопороза, а также фармакогенети-ческих аспектов современной терапии заболевания.
