Surgery treatment remains a basic method in the treatment of vascular obliterans limb diseases. In our days surgical methods are known as the most effective treatment of main artery atherosclerosis, but this kind of treatment is not so good for treatment peripheral arterial diseases such as thromboangitis obliterans (Buerger’s disease) because of often restenosis and rethrombosis development. Principal purpose efforts of surgeons according on ensuring of adequate blood flow in terminal parts of limb and also on support effective perfusion of tissue. This aim could be obtain due to undirected revascularization. Recently thanks to development of cells technologies on a level with osteothrepanation and lumbal sympathectomy it became possible to create new ways of roundabout blood flow in ischemic limb. Autological stem cells transplantation to patients with critical ischemic limb provides an appearance of new sites revascularization. In last years experimental and clinical studies in this area confirm safety and efficiency of the stem cells transplantation to patients with ischemic limb disease. However, few questions about quality and quantity of stem cells which are necessary for appearance revascularization remain unknown.</p></abstract><trans-abstract xml:lang="ru"><p>В статье представлен обзор вариантов применения клеточных технологий в лечении терминальной стадии хронической ишемии нижних конечностей. Основным методом лечения сосудистых заболеваний нижних конечностей остается хирургическое вмешательство. Высокая эффективность применения хирургического метода, достигается при поражении магистральных артерий, в то же время, при поражении дистальных сегментов периферических артерий (облитерирующий эндартериит и тромбангиит - болезнь Бюргера) оперативное лечение мало эффективно вследствие повторного образования тромбов и последующего стенозирования. Основные усилия хирургов при лечении таких заболеваний направлены на обеспечение адекватного притока крови к дистальным отделам конечности и поддержания эффективной перфузии тканей. Этого можно достичь путем непрямой реваскуляризации. Помимо традиционных методов лечения, таких как остеотрепанация и поясничная симпатэктомия, в последнее время активно изучается возможность применения клеточных технологий для создания новых путей коллатерального кровообращения в конечности. Трансплантация больному его собственных стволовых клеток стимулирует ангиогенез в ишемизированных тканях. Проведенные экспериментальные и клинические исследования в этой области подтверждают высокую эффективность и безопасность трансплантации стволовых клеток больным с данной патологией.
